I consent to the processing of the personal data I have provided in the questionnaire — including special categories of personal data within the meaning of Article 9 of the GDPR (e.g., health-related information)—for the purpose of contacting me and assessing my suitability for participation in the study.

I agree that my data will first be transmitted in encrypted form to the sponsor/study management and that an automatic or manual assignment to a suitable study center will be made. For this purpose, the study management may view and process the personal data required for this (e.g., postal code).

I am aware that my contact details and the information provided in the questionnaire will be forwarded to a member of the study center closest to my place of residence or to a third-party service provider.

I may revoke this consent at any time with future effect without incurring any disadvantages. The revocation does not affect the legality of the processing carried out until the revocation.
