I consent to the processing of the personal data I have provided in the questionnaire—including special categories of personal data within the meaning of Article 9 of the GDPR (e.g., health-related information)—for the purpose of contacting me and assessing my suitability for participation in the study.

I am aware that my contact details and the information provided in the questionnaire will be automatically forwarded to the study center closest to my place of residence.

If my place of residence is more than 300 km away from the nearest study center (“out of range”), I agree that my data will first be transmitted in encrypted form to the sponsor/study management and that a manual assignment to a suitable study center will be made. For this purpose, the study management may view and process the personal data required for this (e.g., postal code).

I may revoke this consent at any time with future effect without incurring any disadvantages. The revocation does not affect the legality of the processing carried out until the revocation.
